The state office of Special Olympics requires that we have new forms filled out and on file every three years for every athlete participating in our area. These new forms are included in this packet. Every athlete will need to have these filled out, signed and on file in order to participate in competitions this year. 

Please use the Fax cover sheet in this packet and fax to 866-542-1860 when sending information for Special Olympics. We appreciate your efforts!
